Clinical Masters Menu  

Once templates and Header templates have been created, they must be assigned to a form to appear in the EHR if needed. Forms set the letter head, print format, menus, and templates for each encounter note. Any number of forms can be created for visit types, specialties, providers, and locations.

Add or Edit a Form

To add or edit a form, perform the following steps:

1.      Click Clinical Forms on the Clinical Masters menu and then the New Link to create a new form or click Edit next to the form you want to change.

2.      Then Form Setup screen appears.

3.      Enter or change the Form Name and type a description of form’s purpose.

4.      Select a Visit Type this form will be associated with.

5.      Specialization should be selected if the form is to be assigned to a group of specialists or individual providers.  Specialization for each provider must be set in Administration Provider Details.

6.      Select the owner as either a group or an individual provider if a specialization has been selected.

7.      Facility can be selected to have a form specifically for this facility.  This is to support different letter head for each facility.

8.      Make the form active or inactive.

9.      Speech recognition activates the speech recognition option for this form.

10.   Make default for this visit will automatically load this form for an encounter that matches the settings of visit type, specialty, owner, and facility.

11.   Font type and size define the font style and font size used for all template text and typed content, except for section names and field names, which are configured separately within the template settings.

12.  Set Page size to letter page size.

13.  Set the Top, Bottom, left, and right page margin.

14.   Page Number Format defines the style in which page numbers are displayed on the document. Page No Alignment determines the position of the page number on the page, and Page Number Footer Margin defines the distance between the page number and the bottom of the page.

15.   The Page 1 settings section under Report Header allows text to be entered on the left, center, or right side of the header. A logo can also be uploaded and placed in any of these sections. To upload a logo, click the Choose File button next to Add Logo, select the required file, and then click Upload.

16.    The text cell can be formatted by right-clicking inside the cell and selecting Cell Properties. 

17.   Select a header form and set the font, font size, and margin from the top of the page that it will be inserted.

18.   A header can also be configured for all pages after the first page. Additionally, a separate header form can be assigned for these subsequent pages.

19.   Click the Form Templates tab to assign templates and how they should appear in the note.  Check the checkbox Display in Notes to add the template.

20.   To remove a template from the form, click the Remove Link in the row of the Template’s name.

21.   To move the order of the templates, select the template and drag it to the desired position while holding the left mouse button.

22.   To change the template name, enter a new name in the template name field. To prevent the template from appearing in the note, uncheck the Display in Note checkbox.

23.   Click the Save button to save the form.

Default AI Sections Text: The Default AI Sections Text tab allows administrators to configure the default AI prompt and section structure used for generating clinical notes. This setup defines the categories and formatting instructions that the AI follows while organizing encounter documentation into sections such as Chief Complaint, HPI, PMH, ROS, Vitals, Physical Exam, Assessment, and Plan.

 Steps To Copy A Form

  1. Click Edit to open the form you want to copy.
  2. On the Form Setup screen, click the New button. This clears the form name while retaining all other form settings and configurations.
  3. Enter a new name for the form and click Save.
  4. A copy of the form will now be created with the new name.
