Clinical Masters Menu  

Form Header enables patient and encounter information to be inserted in a table at the top of the first page and/or every other page.  Multiple Form Headers can be created and assigned to Page 1 or subsequent pages of a form.  A maximum of Five columns and Eight rows can be added as a header.

Create or Edit a Form Header Template

  1. Click Form Header on the Clinical Masters menu.
  2. Type the name of a new form or click Edit next to the name of a Form Header Template to edit.
  3. Select the number of columns and rows.
  4. Select if the form header is active or inactive.
  5. Show Border: Displays borders around the header section when the form is generated or printed.
  6. Show Date: Displays the current date in the form header.
  7. Spaceline: Defines the spacing between lines within the form header.
  8. Click the submit button to display the correct number of columns and rows below.
  9. In Column 1, type the field name and select the field name to display in the header.  The field options are: account number, patient name, encounter number, encounter date, facility name, provider name, provider specialty, provider mobile number, provider work phone number, provider home number, provider email, provider electronic signature, patient age, patient DOB, patient gender, patient home phone number, patient mobile number, patient social security number, last appointment, patient email, patient gender/age, next appointment, referring physician, other medical records and primary policy number.
  10. Click the Save button to save the template and add another.
