Templates and consents can be assigned to appointment visit types and sent to patients to complete on the web, mobile phone or in the office on a kiosk.
Consents
Consents are created as note templates so that defaulting information can appear in the note. Consents use a touch screen or mouse to sign the consent. Once Signed is clicked the Consent log is updated and the signed PDF is saved in the attachments folder.
Check-in Form Setup
Select the visit type and then select the templates for the patient to complete. Drag and drop to move the order. To visit Template setup screen, click on Main Menu > Patient Intake > Template Setup.
IN-Office Kiosk
Login and on the Appointment calendar click the Appointment Check-in button. This opens up a new tab. Enter the patient’s last name and DOB, which then checks them in for their appointment.
Problems Form
This form allows users to enter and document the problems or concerns a patient is experiencing.
